Ballarat Hospice Care will be providing awareness building sessions during the week of the 18-24 March at Sebastopol Library as it is National Advance Care Planning Week.
Advance Care Planning is something that everyone should be thinking about regardless of age, health, or capacity.
Advance care planning involves thinking about and communicating your future health care wishes and preferences for a time when you may not be able to communicate. It can also involve documenting these decisions.
If you would like to pick up the paperwork involved in this process and/or discuss with our Hospice Supportive Care Team, how Advance Care Planning might be of benefit to you or your loved ones, please come and say hi to us at Sebastopol Library on the below days:
