Palliative Care Nurse Practitioner (NP)

Ballarat Hospice Care is seeking an experienced and compassionate Palliative Care Nurse Practitioner (NP) to join our dedicated team. This is a clinical leadership role where your advanced clinical expertise will support high-quality patient outcomes and enhance team cohesion.

About the Role:

The Palliative Care Nurse Practitioner (NP) will work closely with Ballarat Hospice Care’s established interdisciplinary team to deliver exceptional palliative care. In this collaborative role, you will:

  • Bring advanced skills in pain management, symptom control, and end-of-life planning.
  • Identify opportunities to implement innovative practices and advancements in palliative care.
  • Respect and build on the team’s existing workflows and extensive experience.
  • Strengthen team cohesion by fostering trust, collaboration, and understanding.
  • Mentor team members, providing guidance on best practices and supporting their continuous professional growth.

This role will include both informal teaching and structured learning opportunities to ensure the team remains at the forefront of evidence-based palliative care.

Key Responsibilities:

  • Provide clinical leadership to enhance team performance and care quality.
  • Deliver best-practice palliative care tailored to individual needs.
  • Uphold the highest standards of quality and safety in patient care.
  • Collaborate effectively within an interdisciplinary team.
  • Mentor and educate staff, carers, and healthcare providers, fostering a culture of continuous learning.
  • Promote cultural appropriateness and respect for diversity in care delivery.

About You:

To be successful in this role, you will need:

  • Registered Nurse qualification with current AHPRA registration.
  • Postgraduate qualification in palliative care.
  • Nursing Practitioner qualification or equivalent, as determined by the Board of Management.
  • Strong knowledge of pain and symptom management, and end-of-life planning.
  • Experience in providing mentorship and education to colleagues and healthcare professionals.
  • Ability to collaborate effectively with an interdisciplinary team.
  • Empathy, compassion, and a patient-centred approach to care.
  • Adherence to the Nursing and Midwifery Board of Australia (NMBA) Code of Conduct.

Also required:

  • Current Police Check (on induction)
  • Current Victorian Driver’s License
  • Fully vaccinated – regarding Covid-19 and Influenza
