Supportive Care Advisor

Part-time (0.6 FTE)

We are seeking a compassionate Supportive Care Advisor to provide direct care and support to patients and their caregivers in their own homes, along the palliative care journey and into bereavement.

You will deliver psychosocial and spiritual assessment, ongoing care planning, counselling, and liaise with services both within Ballarat Hospice Care and the wider community. Working closely with a multidisciplinary team, including volunteers, you will actively contribute to organisational meetings, community events, and education sessions. Some planned out-of-hours activities may be required.

About you

You will bring experience in counselling (or similar), social work or pastoral care, and a commitment to holistic, person-centred practice. You are empathetic, professional, and able to work collaboratively within a multidisciplinary team.

A respectful and inclusive approach to cultural diversity and individual needs is essential.
