Comprehensive Palliative Care in Aged Care (CPCiAC) Project

Funded by the Australian and Victorian Governments, the Comprehensive Palliative Care in Aged Care (CPCiAC) Project focused on developing, piloting and implementing a model of care for the early recognition of, and response to, deterioration in aged care residents. This includes a referral pathway with clear guidance to specialist palliative care services for residents with identified palliative care needs.

Additional project priorities included strengthening collaboration across the broader multidisciplinary care team, including General Practitioners and specialist palliative care services, and supporting aged care staff to build their knowledge, skills and confidence in providing a palliative approach to care.

This region-wide initiative was a collaboration between Ballarat Hospice Care, the Grampians Region Palliative Care Consortium and its member organisations, the Grampians Palliative Care Consultancy Team (Grampians Health), and Central Highlands Rural Health during the pilot phase.
